Filling in the CRB form and Identification Documents

Please open the Guide to completing the application form. The guide also describes the documentation which is acceptable as proof of your identity and the number and type of ORIGINAL documents you will need to produce.

1                    YOU MUST USE BLACK INK

2                    YOU MUST USE NEAT BLOCK CAPITALS TO COMPLETE THE FORM

3                    Leave an empty box between words BUT NOT between POSTCODES OR TELEPHONE NUMBERS

4                    If you still regard your HOME ADDRESS as the address you return to outside term time use that address rather than your term time address

Section A           

5          YOU MUST complete all lines in full except lines 5 and 7 which are optional.

6        If you do not have a National Insurance number write “NONE”.

7         If your title is Ms or Mrs you MUST complete the “surname at birth” line in Section C

DO NOT COMPLETE THIS SECTION. ReachOut! will complete it for you. You are applying to be a voluntary mentor with ReachOut!

Complete all lines that apply to you including relevant dates

Section D

Write down your previous addresses if you have not lived at the same address for 5 years. If there is not enough space on the form for all previous addresses use the format below as a continuation sheet.

Sections E F G X and Y     IGNORE THESE SECTIONS. YOU DO NOT NEED TO COMPLETE THEM

Section H       Complete in full remembering to sign BOTH boxes. If you have unspent criminal convictions you must cross the “yes” box.
